Requests for Verification of State Licensure
License verifications are letters sent to other state agencies to verify the status of a license. These letters may be required to assist the agency in determining if there is any reason to prohibit or delay licensure by alerting state agencies of a disciplinary action(s).
There are two options available to request a license verification:
Online
A license verification may be electronically requested through the licensee’s BreEZe account. From the Quick Start Menu, select Verification Letter Request under Manage Your License Information. The fee is $10, and the license verification is mailed to requesting state agency within 10 business days.
By Mail
A license verification may be obtained by mailing a signed and dated letter to the Physician Assistant Board that includes the following:
- Contact information (phone number, email address) of the person requesting the license verification.
- Complete name of the licensee and the license number of each physician assistant for whom a license verification is being requested.
- Name and address of the state agency where the letter will be sent via U.S. Mail.
- A check or money order made payable to the Physician Assistant Board for each license verification requested, $10 per license verification. Credit card payments are not accepted.
The license verification will be mailed to the requesting state agency within 10 business days of receiving the request.
