1. If I have not been previously license/certified/registered as a health care provider, do I have to request a self-query?

   Yes, this is a requirement for all applicants.

2. Can I request the report prior to submitting my application for licensure?

Yes, submit a request for a self-query report within 30 days of submitting the application for licensure.

3. If I experience issues with completing the self-query or the self-query process, should I contact the Board?

4. If I receive an error message when entering my PA program graduation date what do I do?

Enter the current date as you will not be allowed to enter a future date or leave this field blank.
