Frequently Asked Questions

  1. If I have not been previously license/certified/registered as a health care provider, do I have to request a self-query?

    Yes - This is a requirement for all applicants.

  2. Should I open the report once I have received it?

    No - You are required to forward the original, sealed report to Board by mail - 2005 Evergreen Street, Suite 2250, Sacramento, CA 95815.

  3. What should I do if I open the report?

    Contact the Board by email paboard@dca.ca.gov, or by phone (916) 561-8780.

  4. Are PDF versions of the report acceptable?

    No - The Board will not accept a PDF version of the report.

  5. How long will it take for me to receive the NPDB report?

    7-10 business days.

  6. Can I request the report prior to submitting my application for licensure?

    Yes - Submit the self-query request within 30 days of submitting the application for licensure.

  7. If I experience issues with completing the self-query or the self-query process should I contact the Board?

    No - Please contact the NPDB directly by email, help@npdb.hrsa.gov, or by phone, 1-800-767-6732.
