Application Instructions for Licensure

The Physician Assistant Board (Board) will expedite the application review, with the required documentation, for:

  • Active Duty Military
  • Military Veteran – Honorably Discharged
  • Active Duty Military – Spouses or Partner

Please review the Application Instructions and General Information before completing the application.

Please allow the Board 30 days to complete the application review before contacting the Board for an update.

In addition to submitting the application, the following items must be submitted to the Board by mail:

  1. Form PA6 – Physician Assistant Training Program Certification.
  2. Form PA7 – Verification of Licensure, if applicable. The Board also accepts other states’ verification forms.
  3. Form PA8 – Birth Month Licensure Request, if applicable.
  4. PANCE Score
    To authorize the electronic release of your score to the Board, visit www.nccpa.net. The Board does not require physician assistants to maintain NCCPA certification. Verification letters and PANRE score will not be accepted.
  5. National Practitioner Data Bank Report
    To place a Self-Query order, visit www.npdb.hrsa.gov/SQ. Mail the original, sealed report to the Board. PDF versions of the report will not be accepted.
  6. Live Scan Service form or two (2) completed fingerprint cards.

You are personally responsible for all information disclosed on your application, including any responses completed on your behalf. An application may be denied based upon falsification or misrepresentation of any item or response on the application or any attachment.
